Please print out the Sign Up Form below and bring, along with Registration Fees, to scheduled sign up dates.

(To print form, right click anywhere on the form and then click PRINT PICTURE.  This should set your menu options to print the form.)


FEES:  Basketball $55 For First Child with $5 reduction for each additional child ($50 for 2nd, $45 for 3rd, etc.)
          Baseball $65 for each child
          Football $60 for 1st child and $55 for each additional child.
